Endocrine mucin-producing sweat gland carcinoma (EMPSGC) is a rare, low-grade malignant neoplasm that typically presents as a solitary cystic nodule on the lower eyelid of elderly women. It is histologically and immunohistochemically analogous to solid papillary carcinoma of the breast/endocrine ductal carcinoma in situ with a nodular, papillary/cribriform architecture, neuroendocrine differentiation, and mucin production. Herein, we describe the clinical and histopathologic findings of a 68-year-old female with an endocrine mucin-producing sweat gland carcinoma on the left lateral cheek, an uncommon location. While the literature traces this tumor’s origin to the apocrine glands of Moll along the margin of the eyelid, its occurrence at extraocular sites suggests the possibility of a different cell lineage or adnexal origin.
