Nevus sebaceus of Jadassohn is a rare benign congenital hamartoma composed predominantly of sebaceus glands. Nevus sebaceus presents at birth as a well-demarcated skin-colored to yellowish alopecic patch often in the head and neck region, especially the scalp. While it is well known that nevus sebaceus is associated with secondary neoplasms, most of which are benign, a small percentage may be associated with malignant neoplasms, the most common being basal cell carcinoma. This study highlights an unusual case of basal cell carcinoma arising in a background of nevus sebaceus located at the base of the penis in a young adult. A 25-year-old healthy Hispanic male presented with a 6-month history of a slowly growing wart on the pelvic area. The patient stated he had STD testing completed recently, including blood work, which was negative. Physical examination was notable for a 3 x 2 mm erythematous verrucous papule located at the base of the penis. On dermoscopy, the lesion had shiny white structures centrally. The patient underwent shave biopsy which revealed basal cell carcinoma, superficial, nodular, and focally infiltrative subtypes. The patient subsequently underwent excision which showed residual basal cell carcinoma with a background of nevus sebaceus. While there have been rare cases of basal cell carcinomas arising in nevus sebaceus in younger adults, to our knowledge, no cases have occurred at the base of the penis. This case report highlights a unique case presentation of basal cell carcinoma arising in a background of nevus sebaceus at a special site.
