Pemphigoid is a group of autoimmune blistering diseases characterized by subepithelial blister formation. End-stage renal disease (ESRD) is the final stage of chronic kidney disease where renal transplantation or dialysis is required for long-term survival. Pemphigoid diseases have an association with both ESRD and its treatment. However, little is known about the evidence-based treatment strategy of pemphigoid diseases comorbid ESRD. The present review systematically compiled fifty-five case reports and relevant studies, summarized the underlying mechanisms of pemphigoid diseases in ESRD patients, and concluded the efficacy of various therapies. Triggers removal can help optimize pemphigoid treatment. Despite their suboptimal efficacy, corticosteroids remain the most commonly-utilized agents in these patients’ populations. Mycophenolate mofetil, doxycycline, azathioprine and tralokinumab are less used with limited benefits. Treatment by methotrexate and plasmapheresis are associated with significant adverse effects. Consequently, these findings may enable clinicians to optimize the therapeutic approach for these patients.
