Abstract
Cutaneous epithelioid angiomatous nodule (CEAN) is a rare, recently benign vascular lesion described in 2004. To our knowledge, 65 cases have been reported so far. We describe a case of a 39-year-old Caucasian gentleman without known HIV status having recurrent vascular lesions involving the previous resection site and different sites on his face. The clinical differential diagnosis was pyogenic granuloma. This lesion raises the question about the status of the CEAN - is it truly a distinct entity, or should it be in a spectrum of other vascular lesions like epithelioid hemangioma/ angiolymphoid hyperplasia with eosinophilia (ALHE) or malignant (such as epithelioid haemangioendothelioma and epithelioid angiosarcoma). Besides the distinct histological features, this case shows a degree of cytological atypia, which is unusual for this lesion based on what has been described in the literature. With its displayed propensity to recur and the cytological atypia, this case raises the concern of possible malignant potential necessitating reclassification of this lesion under other vascular entities.
